Evaluation Tools

Below are some tempaltes you can use to evaluated your Youth Engagement Program throughout the process of planning, developing, and implementing it:

  • Activity Report Questionnaire - Use this tool to encourage your adult ally to report on their work with the youth group as often as you feel necessary (i.e. biweekly, monthly, bimonthly, etc)
  • Youth Group Survey - Survey the youth that will be part of your youth group prior to the group's start date to understand what expectations and needs the youth have for the group - en français
  • Baseline Youth Survey - Survey the youth you anticipate will be affected by the youth group''s work (i.e. the youth in a given school, neighbourhood, grade, etc.) to determine needs of that audience.  - en français
  • Questionnaire for Parents - Survey the youth group member's parents to see if they feel the youth group has benefited their child. - en français
  • Adult team focus group discussion questions - Use these questions to guide focus group sessions with the adult allies working with the youth to see what has worked, what neesd to be changed, etc.
  • Youth Group focus group discussion questions - Use these questions to guide focus group sessions with the youth group members to see what has worked, what neesd to be changed, etc.
